Medicare – What’s It All About?

In the U.S., reaching 65 is synonymous with becoming Medicare-eligible. Medicare is a national health insurance program started in 1965. It was created and administered by the U.S. government and is funded mainly by payroll taxes.

At its inception, “Original Medicare” covered hospital stays (Part A) and other expenses like doctor fees (Part B). Over the years, Congress added additional Medicare components:

  • Part C – Also known as “Medicare Advantage,” private insurers provide Parts A, B and D coverage according to government regulations.
  • Part D – Covers prescription drug costs. Private companies provide this benefit.

This brief description is but the tip of the iceberg. There is a lot more information you need to know to be a savvy Medicare beneficiary.

Is Medicare Mandatory?

Most of us have increasing medical needs as we age, so it makes sense to have health insurance. The U.S. government thinks so, too. Therefore, Medicare sign-up is mandatory. In addition, private health insurers depend on their policyholders 65 and older signing up for Medicare, so the insurers don’t have to cover 100% of health claims.

Should I Sign Up for Medicare?

The answers to these four questions can serve as a guide in determining if you should sign-up for Medicare.

  1. Have you turned 65? If so, most people need to sign up for Medicare Parts A and B. However, there are some exceptions.
  2. Are you 65, still working, and covered by an employer health plan? If not, you would be required to sign up for Parts A and B. Otherwise, go to Question 3.
  3. How many employees for the employer providing health insurance? If it is under 20 employees, signing up for Parts A and B is required at age 65. For organizations with 20 or more employees, go to Question 4.
  4. Does the employer’s health plan meet or exceed what Medicare provides? If not, you must sign up for Original Medicare (Parts A and B). On the other hand, if the health insurance plan DOES meet Medicare requirements, it provides “creditable coverage.” This means you can put off signing up for Medicare until employer coverage no longer applies.

How Do I Sign Up?

Most people don’t know that signing up for social security benefits also serves as enrollment for Medicare Part A and Part B. Otherwise, you can either click this link to Medicare.gov and follow the prompts for more information or call Social Security at 1-800-772-1213.

When Do I Sign Up for Medicare?

Initial Enrollment Period

This is a seven-month period that begins three months before the month of turning 65.

Special Enrollment Period

You may not sign up during the Initial Enrollment Period because you are covered under employer health insurance. The Special Enrollment Period lasts for eight months after employer coverage ends.

General Enrollment Period

This is for those who did not sign up for Medicare during the Initial Enrollment Period and are not eligible for a Special Enrollment Period. The General Enrollment Period occurs between January 1 through March 31 each year, with coverage starting on July 1 of that year.

Part D Enrollment

Medicare Part D covers prescription medications, but it is optional, unlike Parts A and B. However, sign-up occurs during regular Medicare Initial Enrollment or General Enrollment if you choose to have this coverage. Note that prescription drug coverage can also be obtained through Medicare Advantage Plans (see below), which have their own enrollment period.

Enrollment in Medicare Advantage Plans

Also known as Part C, private health insurers “bundle” Parts A, B and D according to Medicare rules. Enrollment for these plans occurs annually from January 1 – March 31. Beneficiaries with a Medicare Advantage Plan can switch to a different one or change to Original Medicare during this time.

What If I Don’t Sign Up?

There are penalties for not signing up for Medicare on time. The list can be found at Medicare.gov.

What Does Medicare Cost?

Medicare Part B and Part D both have monthly premium costs, but Part A does not. Also, Medicare coverage has deductibles, copays and coinsurance. Details for all of these costs can also be found at Medicare.gov. Medicare Advantage Plans have their own pricing policies.

One of the problems of Original Medicare (Parts A and B) is no limit on out-of-pocket expenses. These “gaps” in coverage can be expensive. As a result, private insurance companies offer Medicare Supplement Insurance, otherwise known as Medigap coverage. These plans have extra premiums on top of other Medicare premiums but help fill in some of the gaps. Note that Medigap is only available to supplement Original Medicare and cannot be used with Medicare Advantage Plans.

Some people cannot afford Medicare’s premiums, deductibles, copays or coinsurance. However, several programs are available to help pay these costs, like Medicaid, the Medicare Savings Program and the Extra Help program that helps with Part D prescription drug expenses.

Medicare is an excellent program for those over 65, but it can be complicated. Hopefully, this brief introduction throws some light on a complex subject. Therapy at 60 can seem a bit
daunting. There’s a lot of life to talk about. Yet I’ve seen many older people delve
into emotional struggles quite successfully.

Yet therapy might not seem needed to
someone whose life looks perfect. “It’s my time to enjoy life. I wouldn’t
change a thing.” Maybe that’s true. If so, that’s wonderful.

But it’s also true that perfect-seeming
can be far from perfect.

Perfectly Hidden Depression

I’ve been researching what I term perfectly hidden depression (PHD) for several years. If that phrase
brings you relief, or intrigues you, then you may be someone whose secrets have
been hidden by a perfect-looking life.

If you’ve never been able to turn off
a nagging voice of self-criticism and shame, if you don’t know how to connect
with painful emotions, or believe that if your true feelings were known, you’d
lose others’ respect, then you might far too easily identify with PHD.

There are many reasons to break your
silence.

The Outside Doesn’t Match the Inside

The pressure you can feel is immense.
You can’t ever let anyone down because you’re only as important and successful
as your last project.

No matter that you get more tired
than you used to or you’re grieving friends that have died, you have to be the
penultimate grandmother, the consummate advocate for whatever cause you’ve
taken up.

You can be surrounded by friends,
involved in a job or multiple activities where you’re highly engaged – and no
one would look at you and believe you might be hiding shame, anxiety, or fatigue.

Even if you tried to tell them,
they’d likely say, “What, you? You’re the epitome of what every woman wants to
be at 60.” Your own loneliness – knowing that
no one knows the real you – could easily lead to despair.

Are You a Marie?

Let’s talk about Marie. She was 62
when she walked into my office.

I’d met Marie socially many times in
my hometown in Arkansas. When anyone spoke of her, her multiple accomplishments
were touted – having a highly successful legal career; building a non-profit support
program for young mothers with substance abuse issues; creating a loving family
and a network of great friends; even surviving the early death of her husband.

Everyone knew her as a fighter, a
brave heart – someone who cared and who could
influence other people to do the same.

She sat, smiling, in front of me, and
said, “I’ve heard you’re pretty direct.”

I laughed. “Yes, I have that
reputation.”

“Well that’s what I need. Because I
don’t understand, when I have so many blessings in my life, why I feel this
dread. Yes, I’ve had some health issues. But I’m okay now. It’s as if I can never
do enough. I feel panicked that I’m still trying to prove myself. And yet, I
know I have already proven myself. Why can’t I let up?”

When I asked about her past, she looked
away for a second, and then looked back. “I promised myself I’d be honest with you
today. But it’s very hard.”

An hour later, there were tears in
her eyes as she got up to leave. She’d been vulnerable and real. She’d realized
she’d always failed to earn her demanding mother’s love or her absentee
father’s attention.

The coping strategy that she’d
devised to handle her childhood pain, to be extremely accomplished and always
look in control, was now escalating into panic as she grew older. She began to
glimpse the damage done that she’d never allowed anyone, perhaps not even herself,
to see.

Self-compassion began in that hour.
“That’s what my constantly trying to ‘do more’ has meant, hasn’t it? Oh my
gosh. I want to stop. But I don’t know another way.”

What It All Means –
for You

Like Marie, you can choose to risk stirring that pot – to look into the recesses of your heart and mind for hurt and grief that you’ve had “no time for,” and to challenge the rigid rules that still govern you.

Because it is your time. It’s
time for freedom from shame.

It’s time to give yourself the gift
of vulnerability.

And it’s time for genuine
self-acceptance.

Depression Awareness Month: Mental Health Resources Covered by Medicare

October is Depression Awareness Month, depression being a serious mental health disorder that is plaguing Americans now more than ever due to the pandemic, inflation, natural disasters and so forth.

In June 2020, 31% of U.S. adults reported struggling with symptoms of depression or anxiety and 26% reported symptoms of trauma or stressor-related disorders.

This is especially concerning as it relates to older generations, most of whom fall in the high-risk category, and are combatting social isolation as a result to protect themselves, fear for their friends and loved ones, and anxiety over their own health.

Given the strong link between depression and dementia in older adults, it’s important that seniors are aware of and know how to access behavioral healthcare. Medicare provides coverage for a variety of resources to diagnose and treat mental health conditions, which we’ll break down in this article.

What Does Original Medicare Cover?

Part A provides coverage for inpatient mental healthcare at either a hospital or psychiatric hospital. It will also assist with some of the cost for the room, meals, nursing, and other services. However, there is a 190-day limit on inpatient psychiatric care.

If you exceed this limit, there is a chance Medicare will still cover the cost of care at a general hospital. Another thing to note about inpatient mental healthcare, is the potential for Part B costs from doctor services.

Part B will cover most preventative, outpatient mental healthcare, including routine visits with clinical psychologists, psychiatrists, social workers, counselors, and/or other certified mental health professionals, whether in a therapist’s office or in a clinic.

While co-insurance will apply for these routine visits, Part B does provide free annual screenings for depression and alcohol abuse. Another benefit covered by Part B is activity therapy. This includes music, dance, or art therapy, but may require beneficiaries to pay out-of-pocket and then apply for reimbursement.

One thing to keep in mind though is that Medicare has annual limits for mental health provider services.

Medicare payments for outpatient mental health services, including the treatment of mental, psychoneurotic, and personality disorders for people not inpatient at a hospital, is limited to 62.5% of expenses in the calendar year.

This can impact how much beneficiaries pay in co-insurance based on the mental health professional seen since providers will want to make up the difference in cost.

What Do Medicare Advantage Plans Cover?

Medicare Advantage plans are required to cover the same services as Original Medicare. However, how much of the cost of mental healthcare they cover is determined by the carrier of a beneficiary’s plan.

These carriers also determine which doctors are in-network. As a result, Medicare Advantage plans include less than 23% of psychiatrists on average.

There is an alarmingly low number of mental health professionals who accept Advantage plans. As a result, those who are pre-disposed to or currently dealing with mental health issues will have much more options for diagnosis, preventative care, and treatment with Original Medicare.

What About Medicare Supplement Plans?

As with physical healthcare, Medicare Supplements will also cover the cost-sharing left over by Medicare after they pay for the mental health services received.

Will Part D Cover Medications for Mental Health Conditions?

Each Part D plan has formulary, or a list of drugs, that the policy covers. Rest assured that all plans must provide coverage for all antipsychotic and antidepressant medications. These drugs are part of the six protected classes, and if you require a drug that is not available under your plan’s formulary, you have the right to an appeal.

Take Care of Your Mental Health

It’s been a tough year for everyone, and there is no doubt it has taken a psychological toll on many. It’s important to educate yourself on the mental health resources available to you and know when to seek help.

Medicare provides beneficiaries with services to aid in diagnosis, preventative care, and treatment. Therefore, be sure to talk to loved ones when you’re struggling and find the right treatment plan for you. This could be regular therapy sessions, taking an anti-depressant, or simply speaking with a doctor to help with lifestyle changes to alleviate symptoms.
